Elderly Waiver Support Coordination

Volunteers of America has provided Support Coordination to the elderly and to adults with disabilities since 1992. The program helps participants discover, prioritize and achieve desired personal outcomes by helping them choose their own personal goals, set their daily routine, decide where they live, and exercise their rights.

Support Coordinators help participants and their families to develop their own Plan of Care with life goals and skills to be developed. Support Coordinators also monitor service providers to make sure services are provided efficiently and in accordance with the individual's plan of care. The targeted population to receive Support Coordination services are individuals 65 years of age and older. They must meet Medicaid financial eligibility and the criteria for admission to a nursing facility. This program is also offered for people who are age 22-64 who are disabled according to Medicaid standards or SSI criteria.

Referrals are received through the Office of Aging and Adult Services (OAAS). To enroll in services, contact 877-456-1146.
