Home and Community based Services
We provide Support Coordination Services for individuals receiving Community Choices and Adult Day Health Care Waivers. We proudly serve the following regions and parishes:
- Acadiana Region: Lafayette, Evangeline, St. Landry, St. Martin, Vermilion, Iberia, and Acadia
- Shreveport Region: Claiborne, Bienville, Natchitoches, Webster, Red River, Bossier, Caddo, DeSoto, and Sabine
- Baton Rouge Region: East Baton Rouge, West Baton Rouge, East Feliciana, West Feliciana, Pointe Coupee, Iberville, and Ascension
- Alexandria Region: Avoyelles, Catahoula, Concordia, Grant, LaSalle, Rapides, Vernon, and Winn
Support Coordination is a service provided to qualified eligible Medicaid recipients to assist them with gaining the range of needed support services. The support services include medical, social, educational, and other community resources. The goal of services is for the participant to remain living in the community and receiving the support services to do so.
Referrals are received through the Office of Aging and Adult Services (OAAS). To enroll in services, contact 877-456-1146 to apply for Community Choices Waiver or Adult Day Health Care Waiver.
